New Patients & How to Register

We welcome new patients and accept all completed registration documentation after 12.00 pm everyday except Thursday.

You are required to complete our new patient forms.  These can be collected from reception or alternatively downloaded by clicking on the links below:

Please note: 

Can you please ensure that you provide the following:

  • 2 x utility bills (dated within the last three months)
  • NHS number
  • For children a full birth certificate and a record of a childhood immunisations undertaken.

Completed registration forms to be handed into Reception after 12pm on Monday, Tuesday, Wednesday and Fridays.

Unfortunately we do not accept these forms on a Thursday due to half day closing.
